Note: Lunch is guaranteed for registrants registered by Wednesday, October 16 at 5 pm.

• Payment for tickets must be made with a credit card or through PayPal. The name of the person who will be making payment is the Contact Person on the registration form.

• Purchasers/Contact Persons may purchase up to a maximum of 25 tickets in a single order.

• For each ticket the purchaser/contact person is asked to enter a name, parish name, and ministry position. If it is not known for whom the tickets are being purchased, we suggest entering “Catechist One,” “Catechist Two” or “Youth Minister One,” “Youth Minister Two”, etc. be used to distinguish one ticket from another.

• Tickets will be sent to the Contact Person by email. Tickets may then be printed and given to the catechists/youth ministers who will be attending the event.

• Please note there are separate registration portals for each Forum location. Click here to purchase tickets to the Forum at Sacred Heart - St. Patrick Parish in Monroe.

• Tickets are valid only for the location purchased and will not be cross-honored.

• Walk-in registrations are unable to be accepted.
