Walk-in, open to the public Diabetes Support Group Visits facilitated by Pharmacist & Certified Diabetes Educator, Alexis DiGeronimo and Alyssa Gentry, MSN, APRN, FNP-C are available to answer questions you have about diabetes, nutrition and medication management.Participants do not need to be a CHC patient to attend the Diabetes Group Visits.
Depending on your health insurance plan you may need a referral from your primary care provider and may be responsible for a co-payment.
