Are you passionate about health and wellness promotion? Are you interested in making a difference in your community?
On Lok 30th Street Senior Center is offering free training for all those who are interested in becoming a Health Facilitator. As a Health Facilitator, you will be able to lead the evidence-based Diabetes Empowerment Education Program (DEEP) workshop developed by the Midwest Latino Health Research Training and Policy Center at the University of Illinois at Chicago. These small group interactive workshops meet for two hours each week for six weeks and are meant to provide disease prevention and self-management information to older adults who are diabetic, pre-diabetic and non-diabetic.
On Lok’s programs are funded by the Department of Disability and Aging Services, and trained leaders will need to facilitate workshops in English, Spanish or Cantonese. This training is offered at no cost to facilitators, since On Lok will cover certification fees that will enable the facilitator to teach at least two workshops per year.
Those interested will need to attend a three-day training course at On Lok 30th Street. The candidates who successfully complete all three days of the course will be able to teach workshops (two minimum) at various locations throughout San Francisco County. Health Facilitators are guaranteed a flexible working schedule, since they will be independent contractors for On Lok. On Lok will be in charge of planning all workshops around the Health Facilitator’s availability.
The three-day training course will be held at On Lok, 225 30th St 3rd floor, San Francisco, CA 94131 on:
· Wednesday, April 30, 2025, from 9:00 AM to 4:30 PM
· Thursday, May 1, 2025, from 9:00 AM to 4:30 PM
· Friday, May 2, 2025, from 9:00 AM to 4:30 PM
