The Jefferson County Health Center Diabetes Community Group is a welcoming and supportive space for individuals living with diabetes, as well as their families and caregivers. This group offers a chance to connect with others who understand the day-to-day challenges of managing diabetes, while learning helpful tips, sharing experiences, and receiving guidance from healthcare professionals. Whether you’re newly diagnosed or have been managing diabetes for years, this group is here to empower you with knowledge, encouragement, and community.The purpose of the group is to learn, engage in discussions with others, share experiences, tips, and ideas about living with Diabetes or being a care partner, provide mutual support, and foster a sense of community.
