Create a Free Texas Living Will and Medical Powers of Attorney
There are two basic kinds of health care documents that everyone should make—a healthcare power of attorney and a living will. This is an opportunity for you to work with a licensed attorney to create these critical documents at no cost to you!
A “medical power of attorney”, sometimes referred to as a healthcare power of attorney, is a legal document that allows you to designate someone you trust to make healthcare decisions for you if you become incapacitated or unable to communicate your wishes. This powerful tool ensures that your healthcare choices are respected even when you can’t express them yourself, providing peace of mind and protecting your autonomy during medical crises and challenges.
A “Living Will” (also called a directive to physicians and families or surrogates) is a legal document designed to help you communicate your wishes about specific medical treatment you would or would not like to receive in certain situations. at some time in the future when you are unable to make your wishes known because of illness or injury. These wishes are usually based on personal values.
We will have our in-house notary present to execute these documents.
Please come prepared with:
1) Your driver's license or other legal form of identification
2) The names, addresses, and phone numbers of at least two people (a primary and an alternate) you would like to designate as the person to make medical decisions for you in the future. In Texas, this person is known as your “agent.” Most people name a spouse, partner, relative, or close friend as their agent. The most crucial criteria are trustworthiness and dependability. While it is a good idea to have an alternate person, this is not a requirement, and you may create the documents with only one person listed as your agent.
3) Please give thought to your preferences regarding life-sustaining treatments (such as CPR, mechanical ventilation, artificial nutrition), and pain management and care.
