Community Vaccination Clinic Offered at Parkway UCC

Mark your calendars! Parkway UCC will be a host location for Jonestown Pharmacy’s Vaccination Clinic
This clinic will be held on November 28 from 9 am- 12 pm.
Waiver forms will be available from the church office and can be filled out personally in advance, then collected before the clinic date for insurance to cover your vaccination costs.
If you do not have insurance they will
accept cash payments.
�Vaccines available the day of the clinic will be: Covid booster, RSV, Shingles Pneumonia and Flu.
Stay healthy and get vaccinated for you and the ones around you!
Post or Message here with your interest or contact the admin. in the church office with your interest to receive a form to complete in advance so you’re all set when the clinic comes to Parkway UCC.
