Community Focus Group

We’re inviting people with lived experience with chronic conditions (ex. high blood pressure, diabetes, heart disease, etc.), either their own or a loved one's, to share what resources are helpful, what’s missing, and how we can improve support. Your insights will guide future planning and help strengthen our community. Snacks will be provided and participants will receive a $25 Walmart gift card for their time.
