Register at comhealthpro.org/events by July 10-Event on July 17th! As long as you register, you don’t have to pay until the day of with check or cash 🏌️or you can pay online through PayPal or Venmo.
Join us for a fun-filled day of golf at Hickory Sticks in Van Wert! ⛳️ As we raise funds to support meaningful activities and care for individuals that attend the CHP Adult Day Center in Van Wert.
8 a.m. Check-in / 9 a.m. Shotgun Start
$300/Team or $75/Player
Update: As of 6/29 we have 8 teams registered and we need 18 by July 10th.
Meal and Cart Included, Beverage Cart, and Raffles! Longest Drive, Closest to the Pin, Hopper Wagon, Double or Nothing, and Skins Game-Prizes for 1st, 2nd, and 3rd place teams
Cart Sponsor – $300 (1 available)
Logo displayed on or inside golf carts throughout the event
Recognition at check-in and on social media, One golfer registration included, One hole sponsor sign included.
Hole Sponsor – $100 (unlimited available)
Colored sign displayed at one of the holes on the course
