CHIP Chronic Disease Risk Factors and Management Workgroup

A chronic disease is defined as a condition that lasts a year or more, requiring constant medical care, limits daily activities, or both. Chronic diseases such as diabetes, cancer, and heart disease are the leading causes of death and disability in the United States. These are also the primary drivers of the country's $4.1 trillion in annual health-care costs.

CHIP Chronic Disease Risk Factors and Management Workgroup represents a cross-sector collaboration of community partners to address the goals within the Tulsa County CHIP. This health priority encompasses a large number of goals as it includes the Social Determinants of Health.
