Caring for Change: Family Carers, Community and the Future

What?

Over the last 6 years, thanks to our project Caring Communities of Change, we have had the privilege to work with families and services across Wales to identify pockets of excellency when it comes to inclusive caring communities.

Now the project has come to an end; we want to share and celebrate all that learning as we look to the future and use this opportunity as a springboard to explore and improve Health and Social Care in Wales, together.

Why?

This will be an opportunity for Ministers, Local Government, third sectors and citizens to come together to discover the incredible co-operative and collaborative projects and organisations that support carers and people with learning disabilities across the nation and the opportunities these bring.

How?

We will touch upon important themes such as: Short Breaks & Respite, Transitions in care and home life, to enable voice choice & control, and there will be space to have transparent and honest conversations about how to bring change by showcasing some positive examples of good practice.

The All Wales Forum cordially invites you to attend our event Caring for Change: Family Carers, Community and the Future, which is being held in Cabaret, Wales Millennium Centre, on Wednesday, September the 16th 2026, 10am –3pm

How can I participate?

Please register for your free place and let us know if you have any dietary requirements and accessibility needs. Lunch and refreshments will be provided.

After registering, you will receive a confirmation email containing information about joining the event.

If you are a family carer, we can reimburse travel expenses – please get in touch with us if you have any questions.

Disabled Parking is available, but there are limited spaces – please let us know ASAP and we will book it for you.

Dros y 6 blynedd diwethaf, diolch i'n prosiect Cymunedau Gofalu o Newid, rydyn ni wedi cael y fraint o weithio gyda theuluoedd a gwasanaethau ledled Cymru i nodi pocedi o ragoriaeth mewn perthynas â chymunedau gofalu cynhwysol.

Gan fod y prosiect nawr wedi dod i ben; rydyn ni am rannu a dathlu'r holl wersi hynny wrth i ni edrych tua’r dyfodol a defnyddio'r cyfle hwn fel sbardun i archwilio a gwella Iechyd a Gofal Cymdeithasol yng Nghymru, gyda'n gilydd.

Pam?

Bydd hwn yn gyfle i Weinidogion, Llywodraeth Leol, y trydydd sector a dinasyddion ddod ynghyd i ddarganfod y prosiectau a'r sefydliadau cydweithredol anhygoel sy'n cefnogi gofalwyr a phobl ag anableddau dysgu ledled y wlad a'r cyfleoedd y mae'r rhain yn eu cyflwyno.

Sut?

Byddwn ni’n trafod themâu pwysig fel: Seibiannau Byr a Gofal Seibiant, Pontio mewn Gofal a Bywyd Cartref i alluogi dewis a rheolaeth, a bydd lle i gael sgyrsiau gonest am sut i gyflwyno newid drwy arddangos rhai enghreifftiau cadarnhaol o arfer da.
