Recording Consent and Release Form

The Arizona Section of the American Society of Mechanical Engineers I hereby authorize the Arizona Section of the American Society of Mechanical Engineers (collectively Arizona Section) and those acting pursuant to its authority (including other units of the American Society of Mechanical Engineers), guests of the Arizona Section at Arizona Section facilitated events, and media and news organizations documenting Arizona Section events to:

(1) Record my image, likeness, and voice in video, audio, audio-video, photographic, analog, digital, or electronic format, or any other copying or recording medium, still or moving (collectively the “Image”).

(2) Use my name in connection with the Image, and combine the image with other images, text, graphics, sound ,or other items

(3) Use, reproduce, crop, alter, modify, exhibit ,or distribute the Image in any medium (including, without limitation, the internet, web pages, emails, print publications, film, television, radio, video tapes, DVD, CD-ROM, and others) for any purpose that the (Name) Section of ASME, and those acting pursuant to its authority, deem appropriate, including promotional, marketing, or advertising efforts.

I release the Arizona Section and those others listed in the first paragraph from liability for any violation of any personal, privacy, or proprietary right I may have in connection with such use.

I understand that all such recordings, in whatever medium, shall remain the property of the Arizona Section. I have read and fully understand the terms of this consent and release, which contains the entire agreement of the parties.
