Please note: all participants must be registered for the event to be eligible to receive free gifts (while supplies last)

Age: Boys and girls ages 5–14 welcome.

Registration: Begins at 10:30 AM. Children must be registered individually (EX: if you have three children, each must be reigstered separately).

The event will run from 11:00 AM to 2:00 PM. Parents, friends, and family are welcome to stay and watch but are required to be respectful of all volunteers.

FULL LIABILITY RELEASE WAIVER
Any adult filling out this ticket form must agree to the below liability release. Attendees who do not register online will require a parent or legal guardian to sign on site.

ASSUMPTION OF RISK
I understand that participation in this event involves inherent risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all risks associated with my/my child's participation in the Amare Thomas Back to School Drive.

RELEASE OF LIABILITY
In consideration for being allowed to participate, I, on behalf of myself and/or my child, hereby release, waive, and discharge Amare Thomas, event staff, volunteers, sponsors, and affiliated organizations from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur during participation in the event.

MEDICAL AUTHORIZATION
I certify that I/my child am/is physically fit to participate in event activities. In the event of an emergency, I authorize event staff to obtain medical treatment as deemed necessary. I understand that I am responsible for any medical expenses incurred.

CODE OF CONDUCT
I agree that I/my child will follow all event rules and instructions. Failure to do so may result in dismissal from the event without refund.

PUBLICITY RELEASE
I grant permission to the Amare Thomas Back to School Drive and its representatives to use photographs, video, or audio recordings of me/my child for promotional, marketing, and media purposes without compensation.

ACKNOWLEDGMENT
I have read and fully understand this waiver and release. I understand that I am giving up certain legal rights by attending or participating in this event.
