Eye Exam Board Certified Ophthalmologist. Dogs must be over 7 weeks old. No online registration:

Auscultation
Auscultation (Basic): Board Certified Cardiologist. Basic Cardiac forms available at clinic or can
be filled out prior at https://ofa.org/applications/ (print and bring with). Please be sure to
have a copy of your registration handy for all pertinent information. Dr. Andrew Waxman,

Echocardiogram Echocardiogram/Doppler (Advanced): Board Certified Cardiologist. Advanced Cardiac Triplicate
forms only to be filled out at the clinic. Please be sure to have a copy of your registration handy

OFA Hips and/or
Elbows
Any Dog(s) over 80lbs there will be an extra fee for sedation and reversal. If any dog is
uncooperative for x-rays then mild sedation to obtain the best positioning for an extra fee. These
fees would be paid extra to Pleasant Paws Pet Center. Hips only is also offered – see sign-up
sheet. Sedation required for dogs 80lbs and over. Sedation will be done after dog has their eyes
and/or heart exams if signed up.
Patella Manual manipulation of the patella to verify soundness
Thyroid Panel Thyroid profile to be done by Antech. Meets requirements for OFA certification. Results mailed
to directly by lab. We recommends that the dog be fasted (no food) for at least 8 hours prior to
test. OFA recommends not testing during a bitch’s estrus cycle.
Others recommend waiting 2 weeks after vaccination.
Microchip Fi Nano microchip provides permanent identification. QR code provided to follow for FREE
registration
Snap 4DX Test All results within 30 minutes. SNAP 4DX – Detects Heartworm, Lyme Disease (Borrelia
burgdoferei), 2 species of Anaplasma (phagocytophilum & platys) and 2 species of Ehrlichia
(canis and ewingii)
Wellness Blood
Panel
Includes Complete Blood Count (CBC), Chemistry Panel to see Organ Function. Results emailed in
1 week.
Rabies One & Three-year Rabies vaccination. For 3 year, need current proof of vaccine within 1 year.
Brucella Important screening for all breeding dogs. Results emailed in 7 – 10 days
