Advance Directive Workshop

What if a sudden illness or injury left you unable to speak for yourself? Who would speak on your behalf? Would they know your wishes and values?
Advance care planning is about having important conversations before a crisis happens. Join us for a thoughtful workshop focused on helping you better understand your options, reflect on your values and begin making plans for future health care decisions.
During the workshop, participants will:
• Explore health care values and goals
• Learn about future medical care choices
• Discuss how to communicate wishes with loved ones and providers
• Take the first steps toward completing an advance directive
Following the presentation, attendees will have the opportunity to schedule a one-on-one appointment with a facilitator for additional guidance in completing an advance directive.
