But what if you get sick or hurt and you can't speak for yourself? Would your loved ones know what medical choices to make for you? Having an advance directive, like a Health Care Power of Attorney or Living Will form, helps you stay in control. An advance directive lets you write down your wishes for medical decisions in case you can't speak for yourself. Think about it. Talk about it. Write it down. Come to an advance care planning workshop. Trained instructors will talk about advance care planning. They will walk you through the Health Care Power of Attorney and Living Will forms. You will have the chance to have all your questions answered and learn what steps to take next.
Walk-ins welcome. A notary will be present to make all forms official.
