A minimum of 4 participants is required. If fewer than 4 register, it will be canceled, with a notice posted on the ADRC website and Facebook page.
Staff from Upland Hills Health Hospital and Clinics will present information about Advanced Directives. Following the presentation, volunteers will be available to witness your Healthcare Power of Attorney document.
