Advance Care Planning - Hutchinson Senior Center

Advance care planning is an important step to ensure you get the medical care you want, even if family members are making the decisions for you. Advance care planning involves identifying a health care agent who can help with medical decisions and developing a written health care directive – a plan for your future medical treatments.

Our trained facilitators can help you initiate conversations, aid with advance care planning, and communicate wishes to your family, friends and doctor.

Our informational meetings are designed to educate you on advance care planning and offer you the option to schedule a free individual session with a facilitator to develop a personalized advance care plan. If you have a health care directive that you would like to review at the session, please bring a copy with you.
