If tragedy or illness leaves you unable to speak for yourself, advance care planning can help you to receive the medical care that you want and ensure that your loved ones know your wishes.
In the seminar, we'll help you:
-Understand the role of an advance care plan
-Consider what to include in your plan
-Designate a health care representative
Invite a family member or close friend
