Adulting with CHD: A Guide to Transitioning from Pediatric to Adult Care

Focused on adolescents with congenital heart disease (CHD) and their parents, this event will feature a transition-topic scavenger hunt for patients, a heart-healthy cooking demonstration, and several parent-focused sessions led by an adult patient, a psychologist, an adult CHD physician, and our TCH transition team.

The event includes:

• Free admission and access to the entire Health Museum
• Opportunity to interact with the transition and adult CHD teams from TCH
• Opportunity to interact with other patients with CHD and their parents
