A dementia care road map for families, caregivers, and clinicians

By the end of this class, participants will be able to:

1. Distinguish the purpose, eligibility criteria, and Medicare coverage requirements of the four dementia care programs — GUIDE, PACE, palliative care, and hospice — so they can accurately explain the key differences to families and care team members.

2. Match each program to the appropriate stage of the Functional Assessment Staging Tool (FAST Scale), identifying when GUIDE is most effective (Stages 3–5), when PACE becomes relevant (Stages 5–6), and when hospice is clinically indicated (Stage 7A–7F).

3. Identify at least two verified local Madison County resources — including BGAAAIL and Hospice Care Plus — as front-door entry points for initiating care navigation conversations before a crisis forces the decision.

4. Recognize the compatibility rules and enrollment conflicts among the four programs — including why GUIDE and PACE cannot be held simultaneously, and why palliative care is the only option fully compatible at every stage — to prevent care gaps caused by enrollment errors.

5. Describe at least two practical next steps a family caregiver, clinician, or care navigator can take after the class — such as asking about GUIDE enrollment early, requesting a palliative care consult at any stage, or calling BGAAAIL at (866) 665-7921 for a free, no-referral assessment.
