Vendor Invitation!By joining forces with organizations, local businesses and community members, the Santa Clara Valley Wellness Foundation strives to support the health and wellness of our valued community. Our mission is to foster the development of the next generation of healthcare providers within the Santa Clara Valley area by providing opportunities that may not otherwise exist.
As a valued partner, we would like to invite you to join our third annual fall fundraiser. Your booth donation will have a direct impact on our upcoming healthcare scholarship recipients and medical career internship students.

INFORMATION BOOTH $35 (10X10) SPACE VENDOR BOOTH $65 (10X10) SPACE
PLEASE SUBMIT YOUR VENDOR FORM BY AUGUST 21ST
VENDORS ARE REQUIRED TO PROVIDE THEIR OWN TABLE, CHAIRS, AND SHADE AS SOME BOOTH SPACES WILL BE OUTDOORS.
