Join Healthy Sheboygan County for an update on how we’re making a difference in the community through our Community Health Improvement Plan (CHIP). We will be in the Phoenix Room on the UW-Green Bay Sheboygan campus on Thursday October 2nd from 5pm to 6:30pm. Please let us know you'll be coming by RSVPing at this link: https://forms.gle/69xwyHfSe4TXfrer6A CHIP is developed based on the priority issues identified in a Community Health Assessment (CHA). The CHIP then supports active engagement by community members and community organizations in improving the health of Sheboygan County. Healthy Sheboygan County’s current CHIP cycle will run from 2024 through the end of 2028.
Five priority areas were chosen for this cycle based on information gathered in the CHA:
• Positive mental health
• Responsible substance use
• Activity and nutrition
• Access to affordable housing
• Access to affordable, high quality child care
Each of these priority areas has a dedicated action team that meets regularly to complete the work outlined in the CHIP.
