****Families will be notified of their child or children's acceptance into the summer program and the date of MANDATORY orientation.

**** By registering for this program, I agree to the following:

• I hereby give Child First Authority, Inc. (CFA, Inc.) and its representatives and approved partners permission to use my child’s photograph and/or video in publications representing the organization.

• I give my child permission to attend all Child First Authority and The Y of Central Maryland Summer Program field trips . I understand that the necessary precautions have been taken to ensure the safety of my child.

***Students having more than two unexcused absences will forfeit transportation***
