$1000 – Co-sponsor includes, banner with company logo, (1) Foursome.
$250 – Hole Sponsor includes a hole sign
Please make checks payable to Lebanon Volunteer Fire Department, and mail to P.O. Box 51, Lebanon, CT 06249. Or drop off at the department at 23 Goshen Hill M-F During the hours of 9am and 1pm.
