High school and college athlete pre-participation evaluation for the 2026-27 school year brought to you by Golden State Orthopedics and Spine. All fees are donated back to the participants athletic program. To date over $55,000 has been donated.

During registration, you will select your appointment time. Please arrive at your selected time with your school's preferred forms in hand. Your portion of the sports physical form should be completed prior to check-in.
