"Your Voice, Your Choice" Advance Care Planning Workshop

What is Advance Care Planning? An Advance Directive? A Healthcare Agent? A POLST form? You are invited to a workshop on completing an Advance Directive. You will be guided through the process of choosing a trusted decision maker and putting your wishes in writing related to healthcare in the event that you are unable to speak for yourself. All documents will be provided, and a Notary Public will be available to notarize your completed document if desired.

Hosted by the PeaceHealth Advance Care Planning Program.

Duration of presentation: Approx. 90 minutes, with Q & A at the end. Open to PeaceHealth patients and all members of the community.

Direction notes: The seminar is held at Peacehealth SW Medical Center. The Education Center is on the South East end of the hospital campus. Use the Main entrance off 92nd Ave. The volunteer desk to your left can point you to the classrooms.
