Age: boys and girls age 5-13 welcome

Registration: begins at 10:00AM
Camp will run from 11:AM to 12:30PM. Parents, friends, and family are welcome to stay and watch but are required to be respectful of all coaches and volunteers.
Please ensure your camper is appropriately dressed in cleats or sneakers with athletic wear. Water bottles are recommended but not required.

FULL LIABILITY RELEASE WAIVER
Any adult filling out this ticket form must agree to the below liability release. Campers who do not register online will require a parent or legal guardian to sign on site.
1. ASSUMPTION OF RISK

I understand that participation in football and related camp activities involves inherent risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all risks associated with my/my child’s participation in the Kool-Aid McKinstry Youth Football Camp.

In consideration for being allowed to participate, I, on behalf of myself and/or my child, hereby release, waive, and discharge Kool-Aid McKinstry, camp staff, coaches, volunteers, sponsors, and affiliated organizations from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur during participation in the camp.

I certify that I/my child am/is physically fit to participate in camp activities. In the event of an emergency, I authorize camp staff to obtain medical treatment as deemed necessary. I understand that I am responsible for any medical expenses incurred.

I agree that I/my child will follow all camp rules and instructions. Failure to do so may result in dismissal from the camp without refund.

I grant permission to the Kool-Iad McKinstry Youth Football Camp and its representatives to use photographs, video, or audio recordings of me/my child for promotional, marketing, and media purposes without compensation.

I have read and fully understand this waiver and release. I understand that I am giving up certain legal rights by attending or participating in this camp.
