An advance care plan is a written record that includes your wishes, preferences, values and goals relevant to all your current and future medical care needs, including when we are close to death and after we have died. Hopefully our own death is a long way off, and still, it is wonderful to reflect on what makes our days meaningful, what brings us joy, and what our values and beliefs are.
Your plan will be especially helpful if something unexpected suddenly happens or if you become very unwell, as you may not be able to express your wishes and thoughts at that time. Writing down our wishes now - whilst we are well and have the time and clarity of mind to do so - makes things easier for our family who may be asked to make urgent, essential decisions on our behalf. It brings a lot of peace to those who care about us, to know what our wishes and values truly are.
An advanced care plan is a very personal document. This day retreat is not to provide answers. Rather, to offer a quiet, safe space for you to contemplate what is important to YOU in the supportive company of a small group engaging with the same important reflections. When creating your plan with others, you get the benefit of new ideas and viewpoints, as well as support and connection.
I began my own advance care plan after my mum suddenly became very ill and unable to communicate at all, and subsequently died 10 weeks later. As a family, we realised that none of us truly knew what mattered most to her in those last weeks, and what her last wishes may have been. Having gone through the process of writing down my own advance care plan, I would like to support others to do the same.
Most likely you will not complete your plan in this one session - it is advisable in fact to discuss your initial thoughts with your whānau and GP / healthcare team and to take your time. Your plan can be updated an unlimited number of times.
