The Rehab Continuum: Strategies for Managing Complex Injury Claims

Continuum of care is a concept involving an integrated system of

care that guides and tracks patients over time through a

comprehensive array of health services spanning all levels of care.

An effective continuum of care system is coordinated. It balances

available resources and provides a framework that is both dynamic

and responsive to changing needs over time. In the workers'

compensation system, the continuum of care can very often be over

the lifetime of the injured worker, especially those with catastrophic

or complex conditions.

The goal of this seminar is to provide you with a better overall

understanding of some of those important components involved in

assessing long-term claims issues, maintaining health, and

maximizing independence of Minnesota's seriously injured workers.
