What is holding service design back from fulfilling its true potential and how do we overcome these barriers?

It can be hard to work out why great ideas don't get implemented, harder still to do something about it. Discussions about the future of service design tend to focus on new technology, yet most organizations still struggle to deliver services that work well for users and staff with the existing technologies we have. Why do we struggle to deliver services that work, and what exactly can we do about it when embracing the right technologies is more important now that ever?
