Release and Waiver of Liability: I understand and release liability from Saint Reed Missionary Baptist Church, the Pastor, officers, and any representatives on behalf of the Church. I understand Saint Reed M.B.C. will do everything according to the CDC (Center for Disease Control and Prevention) and The Los Angeles County Health Department’s safe worship guidelines while inside or outside on church property such as taking temperature before entering, wearing face-covering at all time, practicing social distancing (staying 6 feet apart), and not singing without a face covering. I clearly understand and Accept Full Responsibility and Risk if I DO NOT abide by any AND all rules and safeguard set by the CDC and the Los Angeles County Health Department that Saint Reed M.B.C. and representatives on behalf of the church.

ASSUMPTION OF RISK: I understand and forever release Saint Reed Missionary Baptist Church AND any representatives on behalf of the church from any AND all claims, demands, actions, past, present, or future occurring out of any AND all illnesses, damages, or injuries while participating in all church-sponsored services or activities.

PHOTOGRAPHY RELEASE: I understand that Saint Reed Missionary Baptist Church will live-stream during church services and I grant and give my permission to Saint Reed M.B.C. AND any representative on behalf of the church all rights, title, and interests of any and all photographs, images, videos, and audio in connection when attending all services, events and participating in any way at Saint Reed M.B.C.

BY SIGNING BELOW: I express my understanding AND permission AND entrusted to Saint Reed Missionary Baptist Church by entering the church building. I Release and Waiver of Liability willing AND I am voluntarily providing true and correct information below. This RELEASE AND WAIVER OF LIABILITY FORM is in effect upon making reservations or personal attendance.

REQUIREMENT FOR ATTENDANCE:

• Require pre-registration, mandatory face mask, proof of vaccination, or a negative Covid-19 test within 48 hours of attending church service.

• We ask that all attendees self-check prior to attending in-person service and that they are free of any COVID-19 symptoms, and not be under isolation or quarantine orders.

• St. Reed Missionary Baptist Church has put in place preventive measures to reduce the spread of COVID-19, but cannot prevent you and/or your children from becoming exposed to, contracting, or spreading COVID-19 while attending St. Reed Missionary Baptist Church worship services

• While in attendance, we will practice hand sanitizing and safe physical distancing, and mask-wearing is mandatory at all times.

• We will utilize the Eventbrite platform for pre-registration at each worship service to help with the mandated contact tracing.

• Attendance will be verified upon arrival by an assigned committee using pre-registration documents at the check-in/welcome area.

Our 10:30 AM worship service will continue streaming VIA "Facebook Live" on St. Reed Missionary Baptist Church Facebook page. Service will be available for the listening audience by Conference call at 1-667-770-1351 - Access Code: 500836.
