Calling All High School Athletes!Planning to participate in a sports program? This opportunity is for you! Get a no-cost sports physical at the Care Mobile!
Steps to Follow:
1. Complete Your Paperwork: Print, complete, and sign the following documents:
- Care Mobile Consent
- UPMC TPO
- UPMC Release of Information
- PIAA Sports Physical form
2. Bring Documents to Your Appointment: All forms can be found here www.pghschools.org/sportsphysicals.
3. Register One Week Before the Clinic Date: Register here https://forms.office.com/pages/responsepage.aspx?id=Ptc9i3JOeUaxkVbaFYhxK3N084kJev9JoQQDfR6YmTtURUtIRTBWTklSUzE0V1FUVEFEQUhMTzBSSy4u.
The last student must be registered by 1:00 pm.
4. Attend the Care Mobile Clinic: Students can also get required immunizations at the Care Mobile.

- July 18th - Pittsburgh Carrick, 125 Parkfield St, Pittsburgh PA 15210
Time: 9:30 am – 1:30 pm
