Organization Description:
We are a professional pharmacy fraternity with a purpose of giving back to the community through professionalism and pharmacy service. We give back to the community by volunteering at health fairs, hosting blood drives on campus, helping out the DEA with Drug Take Back Day in San Joaquin County, and having medical international trips to developing countries, and various other forms of service. We are part of the Thomas J. Long School of Pharmacy at the University of the Pacific. The W-9 form attached is from the school.
