About this Event

Charting is one of the most high-stakes parts of your job. It protects your patients, protects you, and tells the story of the care you provided. But most nurses never get the chance to actually practice it before they're doing it for real.

This workshop changes that.

The Real-World Skills & Readiness Lab: Documentation Edition is a 4-hour interactive, case-based workshop designed to help nurses practice clinical documentation in a low-stakes environment. You will work through continuous, evolving scenarios that mirror what you actually encounter on shift. As situations change, so does the charting required.

🕑: 10:00 AM - 10:30 AM
Welcome & Introduction
🕑: 10:30 AM - 11:15 AM
Scenario-Based Charting

Info: The shift begins. Participants receive the opening clinical scenario and begin documenting in real time. Initial assessment findings, care provided, and early shift entries. First chart conflict introduced. Debrief: what was captured, what was missed, and why it matters.


🕑: 11:15 AM - 12:15 PM
Clinical Records & Orders

Info: The scenario continues. New orders arrive. Participants practice MAR charting and MD order transcription within the evolving context. A critical value is flagged. Late entry and addendum documentation applied. Review of common errors and how to correct them without compromising the record.

Documentation Principles Under Pressure

Info: An incident occurs later in the shift. Participants review documentation by exception, person-centred language, and incident/occurrence reporting to a situation that requires both accuracy and care. Patient rights regarding their chart are addressed. Legal pitfalls discussed in context. Terminology review woven throughout. Resources distributed.
