Prevention of poor outcomes from heart disease
eMbedding heAlth equiTy (MATCH) is a programme of work being tested by the City and Hackney Population Health Hub that aims to support teams, services, organisations and communities to improve equity in outcomes, with focus and resources for different groups proportionate to their level of need.
A five-step process is being trialled which involves: (1) bringing people together, (2) collating data and insights to identify needs, (3) using different ‘lenses’ to review what we know to generate change ideas, (4) going through a prioritisation process to identify key change ideas, and (5) implementing change ideas, testing and learning.
Prevention of poor outcomes from heart disease is one of the programme areas which is being taken forward as part of the MATCH programme. We want to improve outcomes for African and Caribbean heritage communities, as we know there are worse outcomes for these groups at the moment.
We are holding two consecutive workshops on Thursday 9th May, 10:45-13:00 and Wednesday 15th May, 10:45-13:00. The workshops will both be held at the Pembury Community Centre (1 Atkins Square, Dalston Ln, Lower Clapton, London E8 1FA). We will provide refreshments and opportunities for interactive discussions.
Workshop 1 will focus on reviewing what we know and agreeing what should be the priority for areas to change and Workshop 2 will focus on generating and prioritising change ideas.
