This session will strengthen your confidence in supporting young people prepare for adulthood from Year 9 onwards, exploring progression pathways, SMART PfA outcomes, effective annual review practice, and how early planning leads to better long‑term outcomes for young people with SEND. Packed with practical tools, real case studies, and local Bradford context, the session will support you to embed high‑quality, person‑centred PfA practice into your everyday work.
