PHOTO RELEASE:

By participating in Pilates Playdate, I understand that photographs and/or video may be taken of me and/or my child(ren) during the event.

I give Peri LLC & ATL Pelvic Health permission to photograph and/or record me and/or my child(ren), and to use those photographs and videos for promotional and marketing purposes, including on Peri’s website, social media, email newsletters, printed materials, and other business-related communications.

I understand that I will not receive compensation for the use of these photographs or videos. I also understand that Peri will not sell my image or my child’s image to third parties.

I may withdraw my permission for future use of photographs or videos by contacting Peri in writing. I understand that Peri cannot be responsible for removing photographs or videos that have already been published or shared before my request.

By purchasing a ticket for this event, I confirm that I am 18 years of age or older and have the authority to provide this consent for myself and, if applicable, my child(ren).
EXERCISE RELEASE:

I understand that participation in Pilates and other physical activity involves inherent risks, including the possibility of injury. I voluntarily choose to participate in this Pilates Playdate and assume responsibility for my participation.

I understand that pregnancy and the postpartum period can involve physical changes and conditions that may affect my ability to safely exercise. I am responsible for determining whether participation is appropriate for me and for following any recommendations or restrictions provided by my healthcare provider.

I understand that the instructors and organizers of this event are not providing medical advice, diagnosis, or treatment. I agree to listen to my body, modify or stop an exercise if needed, and communicate with the instructor about any concerns or limitations.

I represent that I am physically able to participate in the activities offered at this event, or that I have obtained appropriate medical guidance regarding my participation. I agree to notify the instructor if I experience pain, dizziness, bleeding, shortness of breath beyond normal exertion, contractions, fluid leakage, or any other concerning symptom during exercise.

To the fullest extent permitted by law, I release and hold harmless Peri LLC & ATL Pelvic Health, its instructors, contractors, event partners, and affiliated organizations from claims arising from my voluntary participation in the event, except to the extent caused by their gross negligence or willful misconduct.

By purchasing a ticket for this event, I acknowledge that this release applies to my participation in the Pilates Playdate and that I am responsible for making informed decisions about my own health and participation.
