Partner Massage - Learn from an LMT

Join us to learn from the best! Jessica Tolliver, Master Massage Therapist will be hosting our Partner Massage class.

This class is for everyone! Bring your friend, spouse, partner or even feel free to come alone, and learn the basics of massage therapy at home.

Jessica will lead you through some standard techniques and show you and your partner ways to improve your at home massage skills! This will be a fun and informative class that leaves you feeling confident that both you and your partner can help each other to feel better.

By buying a Ticket you agree to the following terms and conditions.

By participating in any Class/Workshop, offered by Massage Arkansas, you understand that during which you will receive information and instruction about yoga and health. You recognize that yoga may require some physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.

You understand that it is my responsibility to consult with a physician prior to and regarding my participation in any class or Workshop. You represent and warrant that I am physically fit and you have no medical condition which I have read the above would prevent my full participation in the Class/Workshop.

In consideration of being permitted to participate in this Class or Workshop, you agree to assume full responsibility for any risks, injuries or damages, known or unknown, which you might incur as a result of participating in the program.

In further consideration of being permitted to participate in this Class/Workshop, you knowingly, voluntarily and expressly waive any claim you may have against Massage Arkansas , or it’s owners, and class/workshop Sponsor, for any injury or damages that you may sustain as a result of participating in the program.

You, your heirs or legal representatives, forever release, waive, discharge and covenant negligence or other acts.

Make sure you have read the above release and waiver of liability and fully understand its contents as well as the

Refund/Cancellation and Make-Class Policies. You voluntarily agree to the terms and conditions stated above.

The World Health Organization has declared the novel Coronavirus (COVID-19) a worldwide pandemic. Due to its capacity to transmit from person to person through respiratory droplets, the government has set recommendations, guidelines, and some prohibitions which Massage Arkansas (the "Organization") adheres to comply

In consideration of my participation in the foregoing, the undersigned acknowledge and agree to the following:

• I am aware of the existence of the risk inherent to participating in a hands-on course and that

• my participation will make social distancing difficult

• I have not experienced fever, fatigue, difficulty in breathing, or dry cough or exhibited any other symptom relating to COVID-19 or any communicable disease within the last 14 days.

• I have not tested positive for COVID-19 within the last 30 days.

• I have not, nor any member(s) of my household, traveled by sea or by air, internationally within the past 30 days.

Following the pronouncements above I hereby declare the following:

am fully and personally responsible for my own safety and actions while and during participation in this course and I recognize that I may be at risk of contracting COVID-19.

With full knowledge of the risks involved, I hereby release, waive, discharge the Organization, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to

COVID-19.

By purchasing a ticket acknowledge that I have read the foregoing Liability Release Waiver and

understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.
