14:00 – 15:00 - The road to population health management - a maturity journey

Shared care record programmes are starting to focus more on population health management. It can be daunting to know how to navigate this next phase whilst continuing to extend your existing ShCR.

Whilst this strategy has been driven by NHS England, we have identified it as a theme common to many of our UK customers. In this session, The Orion Health product team will outline Orion Health’s population health management maturity model, aligned to NHS England’s building blocks of Infrastructure, Intelligence and Interventions, intended to make this progression clearer. As part of this maturity model, the team will discuss unlocking the power of the data through your shared care record to better understand your population and focus on care coordination through solutions such as care planning and transfers of care.

New Zealand’s largest private healthcare organisation and Orion Health development partner will present on their digital transformation programme including recent work to enable vitals capture on-the-move through Clinical Portal Mobile and the implementation of patient forms for digital admissions.

How has the adoption and co-development of the Discover analytics solution helped the CHIE team monitor usage of the system? A look at the ways in which Discover has enabled greater clarity on patterns of system usage, demonstrated improved uptake and highlighted areas for future focus, post-transition onto the Orion Health platform in 2021 and the subsequent introduction of multi-factor authentication for portal access.
