Finding the Right Support, the Right Services, and the Right Person to Call

Trying to care for a loved one can feel overwhelming, especially when you're navigating hospitals, doctors, IHSS, transportation services, home care agencies, and community programs for the first time.

Join CCI, for the first session of our caregiving workshop series as we break down San Francisco's care system and help caregivers understand where to go, who to call, and how to get the support they need.

Participants will learn how local services work, including IHSS, Homebridge, Adult Day Health programs, healthcare providers, transportation services, and community-based organizations. Together, we'll explore common caregiving scenarios, practice navigating phone systems and online portals, and learn strategies for connecting with real people who can help.

• Practice communicating with service providers and advocating for your needs
• Identify key members of your caregiving support team
• Create a personalized Care Map to guide future caregiving decisions

Care Journey Mapping
Walk through a real-life caregiving scenario and identify the people, services, and supports involved from hospital discharge to home care.

"Get to a Human" Practice
Role-play common phone calls and learn practical communication strategies for navigating agencies, clinics, and service providers.

Each participant will receive a personalized Care Map worksheet to help organize important contacts, services, and support resources.
