About this Event
Successive governments have been clear about what health and care services should look like: co-ordinated, personalised, responsive, and delivered much closer to home, in settings such as community trust hospitals, pharmacies and community diagnostic centres. However, this long-held vision is still far from being achieved, and in England the system remains centred around hospitals and emergency care.
Following the launch of a major new report from The King’s Fund, Making care closer to home a reality: refocusing the system to primary and community care, this conference will explore what action is needed across primary, secondary and community care to make care closer to home a reality, and how to develop effective pathways to support better patient outcomes.
Through presentations, panel discussions, and looking at case studies, this conference will explore:
- how working in partnership with others across the system, industry, and the voluntary, community and social enterprise (VCSE) sector can enable organisations to think differently about how they plan and deliver services
- learning from leaders from geographical or clinical areas where shift of focus has been achieved and the benefits of this are being seen
- the relationship between primary and secondary care sectors and how this can help, instead of hinder, the shift that is needed
- how better use can be made of NHS estates to enable the shift of care closer to home
- how to equip the workforce of the future and today to deliver the vision
- how moving care closer to home can be funded
- the role that secondary prevention can play
- how to meaningfully engage the community.
Throughout the day, you will have time to attend and contribute to the conference sessions, as well as network with attendees and exhibitors.

Can I pay by invoice?
Yes. Please select your tickets and once you're through to the registration form, you can amend the 'payment method' from credit card to 'pay by invoice' . Please note, there is a £20+VAT surcharge to cover the cost of processing invoices. This is an addition to the ticket price and will be added to your invoice. There is one invoice fee per order, not per ticket. There is no charge for paying by debit or credit card.
