We kindly request that attendees pre-organize their tables (10 people MAX per table) and fill out the necessary information on the form in the link below or in our bio.
Note: If tables are submitted with less than 10 attendees, MDSS may be required to combine tables. Gather your friends and secure your spot!

Submissions for completed forms will commence on Sunday, 26th May at 7 PM via a Google Form (to be posted). Only one submission is required for each table. The primary contact should be the one to submit and will be the point of contact between MDSS and the table.
To be eligible for the Early Bird rate, your form **must** be submitted by **June 30th, 11:59PM**
