CareSource and LN will be hosting an immunization clinic at LN on April 10th between 6 PM - 8 PM. Walk-ins the day of the clinic are welcome, but we MUST have parent/guardian consent.Please upload or send the immunization records for the individual who is getting vaccinated.The online registration form can be completed at patients.vaxcare.com/registration. Enrollment code is IN65942. Please select Lawrence North H.S.-5/2/24
Please bring your insurance cards. CareSource accepts and bills all insurance (commercial, Medicare, Medicaid). There is no cost to those without insurance, except for the Shingles and adult RSV vaccines, as there is often a required co-pay.
All CDC recommended vaccines will be provided. We will consult
the state immunization registry to assess the immunization needs
of each individual. If you would like to refuse a specific vaccine,
please contact the Indiana Immunization Coalition at least 48
hours prior to the event at [email protected]
