This four part webinar series explores the challenges and complexities of living with multiple chronic conditions, focusing on the interconnected nature of these diseases and the role of prevention, management, and health equity.

Webinar One: Risk factors that lead to chronic conditions, and ways to mitigate these risks through following a healthy lifestyle, 29th January 2025, 13:00-14:00

Preventing and managing chronic conditions begins with addressing their root causes. This session will explore the modifiable risk factors that contribute to chronic diseases, including smoking, poor diet, physical inactivity, and stress. Attendees will gain actionable advice on adopting sustainable lifestyle changes to reduce the burden of chronic conditions

Webinar Two: Heart Failure & Chronic Kidney Disease, 27th February 2025, 12:00-13:00

Heart failure and chronic kidney disease often coexist, creating a complex web of interdependent risks. This session will delve into the shared pathophysiology of these conditions, strategies for early identification, and approaches to integrated management. We’ll discuss the importance of a multidisciplinary approach to improve patient outcomes.

Webinar Three: Obesity, MASLD (fatty liver disease), Diabetes, 24th April 2025, 12:00-13:00

Obesity, MASLD, and diabetes are intricately linked, forming a triad of chronic conditions with significant metabolic and cardiovascular implications. This session will focus on understanding the progression of these diseases, current treatment approaches, and the role of lifestyle modifications in breaking the cycle of metabolic dysfunction.
