Karate Release and Waiver of Liability

Please read carefully before signing. By participating in karate classes, drills, or activities.

I acknowledge and understand that:

• Karate training involves physical activity and potential for injury, including but not limited to sprains, strains, fractures, concussions, and other injuries.
• I am fully aware of the risks associated with participation in karate activities and voluntarily choose to participate despite these risks.
• I understand that I am responsible for my own safety and well-being during karate training.
• I agree to follow all instructions and safety guidelines provided by the instructors and staff.
• I agree to inform the instructors of any pre-existing medical conditions or injuries that may affect my ability to participate safely.
• I hereby release, waive, discharge, and covenant not to sue its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to my participation in karate activities, including, but not limited to, any injuries, damages, or losses that I may sustain.
