Ruff Haven Crisis Sheltering will be hosting a free vaccination clinic for dogs and cats, with the generous sponsorship of Petco Love , Humane Society of Utah, and the Banfield Foundation, as well as veterinary partners Best Friends Animal Society, and Canyons Veterinary Clinic.
Vaccinations will be given by appointment only. Every pet will require its own appointment. Please fill out this form for each pet needing vaccination.
Included in the vaccination clinic are:
- 1-year Rabies vaccine (Animals must be 16 weeks old to receive)
- DHPP (yearly core vaccine for dogs) (Must be 6 weeks old)
- FVRCP (yearly core vaccine for cats) (Must be 8 weeks old)
- Microchip
- Bag of pet food
All animals must be on a leash or in a carrier.
**Please note we are unable to serve community cats.

Las vacunas se administrarán solo con cita previa. Cada mascota requerirá su propia cita. Complete este formulario para cada mascota que necesite vacunación.
