Attention 6th – 12th Grade Youth and Parents,The Choice Pass program is a grant funded incentive-based program that offers alternative activities, partners with community business to offer discounts for youth, and helps youth abstain from using substances while encouraging and celebrating their healthy choices.
Parents, the Choice Pass is a tool for you to help support your child in making healthy decisions, become educated about topics relevant to raising an adolescent, and have a network of supporting families.
How does it Work? Enrollment happens annually. It is open starting 9/7/2021.
1. Visit www.choicepass.net/registration to complete the parent and youth online surveys. Parents only need to complete once.
2. Complete the registration (pledge and waiver), the link is provided at the end of the youth survey.
3. Attend one virtual or in person sign-up night with your child. Each night will have a 6pm or 7pm session. If you are attending virtually, you will join via a zoom link. Links are post below and on www.choicepass.net/registration.
We encourage 6th graders and families new to the program to attend in person. Free ice cream will be provided at in-person nights!
4. If you cannot attend a sign-up night contact GCSAPP prior to the sign-up nights.
ALL YOUTH AND PARENTS MUST ATTEND ONE SIGN-UP NIGHT TOGETHER.
Attend a 6pm or 7pm session. All session will be offered in English and Spanish.

El programa Choice Pass es un programa basado en incentivos basado en donaciones y que ofrece actividades alternas, se asocia con empresas y negocios de la comunidad para ofrecerle descuentos y ayuda a los jóvenes para abstenerse al consumo de sustancias mientras los alienta y celebra en sus decisiones saludables.
Padres, el Choice Pass es una herramienta para ayudar y apoyar a su hijo a tomar decisiones saludables, también a educarse sobre temas relevantes para el desarrollo de un adolescente y tener una red de apoyo a su familia.
¿Como funciona? La inscripción es anual.
Visite www.choicepass.net/registration para rellenar las encuestas en línea para padres y jóvenes. Los padres solo necesitan rellenarla una vez.
1. Complete el registro. El enlace se proporciona al final de la encuesta de jóvenes.
2. Complete el compromiso de Choice Pass en línea.
3. Asista a una noche de inscripción virtual con su hijo. Te unirás a través de un enlace utilizando zoom para conocer más acerca del programa. Cada noche será de 6pm o 7pm cada sesión. Los enlaces se publicarán en www.choicepass.net/registration.

4.Si no puedes asistir a la noche de registro por favor contacta el GCSPP con una noche de anticipación.
