Join Wise & Well for Hospital Discharge Planning, an educational session focused on helping older adults and caregivers navigate the transition home safely and confidently after a hospital stay.

Participants will learn:

• how to prepare the home for recovery

• Medic*tion and follow-up planning tips

• warning signs that require additional support

• strategies that can help reduce fall risk and readmission
