Join us for an engaging and informative session focused on helping infants and families successfully transition from the NICU to home. This event will explore the essential components of discharge readiness, including physiologic stability, caregiver preparedness, and strategies for identifying common post discharge complications.
Participants will gain practical skills for supporting families through feeding challenges, safe sleep education, Medic*tion administration, warning sign recognition, and the importance of structured follow up and developmental monitoring.
Designed for nurses, healthcare providers, and care coordinators, this session emphasizes evidence-based approaches that enhance family confidence, improve infant health outcomes, and reduce preventable readmissions. Education of nursing staff for transitioning NICU graduates to home has been shown to improve outcomes and decrease readmission rates, making this training a vital investment in high quality neonatal care. Join us to strengthen your practice, support safer discharges, and ensure NICU graduates and their families thrive at home.

Registered nurses and other interested healthcare professionals
Objectives
At the conclusion of the educational activity, participants will be able to:
1. Describe physiologic readiness for NICU discharge
2. Identify key caregiver education needs
3. Recognize common post-discharge complications
4. Explain importance of follow-up and developmental monitoring
Participants can earn 1.0 contact hour of nursing continuing professional development credit. Each nursing participant must be present for the entire session and complete a session evaluation to receive credit.
