This informative session will help community members better understand how to reduce fall risks and maintain a safe, supportive home environment. Rehab professionals from The Elliot VNA, including a Physical Therapist and Occupational Therapist, will guide participants through practical strategies for preventing falls, recognizing common hazards, and responding safely if a fall occurs.
Attendees will learn how to assess their home for potential risks, incorporate simple strength and balance techniques into daily routines, and use assistive devices appropriately to support safe mobility. The presentation will also cover best practices for navigating transfers, showering, and bed mobility, along with tips for maintaining independence and confidence at home.
Whether you are supporting an aging loved one or looking to enhance your own safety, this session will provide valuable insights, expert guidance, and actionable steps to help prevent falls and promote well‑being.

Meet the Presenters
Scott R. Plourde, PT, MPT
Scott R. Plourde, PT, MPT is a Home Health Clinical Manager and Physical Therapist with The Elliot VNA. He manages a multidisciplinary team of skilled nurses, occupational and physical therapists, medical social worker, and licensed nursing assistants who provide services to patients in their home. He oversees the care delivery for 175-200 patients in Southern NH. Prior to Scott's transition to his current management position, he worked for several years as a home health physical therapist performing home safety evaluations, implementing fall prevention programs to keep community patients safe.
Alicia Wood OT/L
Alicia Wood, OT/L is a licensed Occupational Therapist with The Elliot VNA. Alicia works with patients to provide education and recommendations to maximize their independence and safety in the home. She accomplishes this through training on equipment and adaptive devices so patients can perform at their highest level of independence.
